Hepatic Abscess in Korea

Abstract
Even though the incidence of hepatic abscess was decreased following adequate use of chemoth
erapeutics and antibiotics during last decade, it¢¥s still remaining high incidence of morbility in
Korea.
Because many patients are seen after a long period of neglect, poorly understanding of it¢¥s
diagnosis and treatment, inadequate use of antibiotics, the treatment was not effective and needed
long term admission.
As the sense of dysentery in Korean was poor, the early treatment was inadequate, so the inci
dence of hepatic abscess was highly developed to secondary complication of the dysentery.
If the physician was recognized the condition in time to given adequate early treatment, could
prevent the patient¢¥s death.
The purpose of this study is to provide a statistical analysis of this problem and a knowledge
of the clinical behavior of hepatic abscess in Korea, there with providing material which can be
used in comparing similar studies made in other countries.
The 45 cases were admitted to the Woosok Medical College Hospital from April 1, 1965 to
March 31, 1970 because of hepatic abscess who were confirmed by surgery and aspiration.
1) Among 45 cases, 24 cases were amebic abscess and 21 cases were pyogenic abscess.
2) The sex ratio was male to female in 2:1, peak age incidence came in 30 to 40 years.
3) The right lobe of liver was involved in 80%, 4 times more than left lobe.
4) The most common causitive agent of pyogenic abscess was E. Coli(33%) and known
causitive disease was cholecysititis (33%).
5) Only 25% of the amebic abscess showed E. Hitolytica in the abscess, the most common
causative disease was amebil dysentery.
6) 86% of cases with pyogenic abscess showed abnormal liver fuction, 92% of cases with
amebic absces were found to habe hepatic dysfunction.
7) 81% of cases in pyogenic abscess had positive X-ray findings and in amebic abscess positive
were present in 82%.
8) 72% of pyogenic abscess and 46% of amebic abscess were treated by open drainage.
9) 10% of pyogenic abscess and 25% of amebic abscess cases were developed complications.
10) The mortality of pyogenic abscess was 5%, 8% in amebic abscess and over all mortality of
hepatic abscess was 6.6%.
